Responsibilities
Shift: Casual Part-Time, Varied Weekdays

Job Summary:
Under the direction of the Wound Care Center Director, the LPN performs duties as directed by a Registered Nurse, or Provider by applying professional nursing theory and principles, and utilizes the nursing process to assess, plan, implement and evaluate care given to patients in the Wound Care Center.
Essential Functions:
Assesses the patient's condition and nursing care needs, and reports complex care needs and/or changes in condition to the RN or Wound Care Provider when applicable.
Provides direct care in accordance with the prescribed medical and nursing treatment plans under the instruction of the Primary RN, Wound Care provider
Communicates with staff and providers to ensure quality of care.
Maintains and demonstrates competency in BLS (Basic Life Support), infection control, safety and all unit required skill review.
Implements and evaluates appropriate patient and family education per Plan of Care and under RN supervision.
Functions as an advocate for the patient and his/her family by supplying and interpreting information, supporting decisions, and facilitating communication.
Establishes and maintains collaborative communications with physicians, other health care professionals, and facility departments.
Completes all visits and documentation within regulatory and Agency timeframes.
Participates in the maintenance of a clean and safe environment.
Responsible for room cleaning between patients.
Supports team members and cooperates well with other staff members, physicians and departments. Maintains positive working relationship and fosters a cooperative work environment. Displays honesty, and mutual respect when communicating with peers and departments.
Qualifications
License/Certification:
Experience:
Two (2) years of experience as an LPN required.
Medical, Surgical, Home Health or Wound Care experience preferred.
Must have basic word processing and spreadsheet skills.
Education:
High school diploma or GED required.
Wound Care/Hyperbaric Management course involving travel required within six (6) months of hire.
